The Jayden DeLuca Foundation is proud 

to announce our

Patient Assistance Fund Program!

 

The Patient Assistance Fund is designed to assist families with children that have been diagnosed with Congenital Heart Defects (CHD). Individuals should document they are a member of a community area served by the Foundation.

Financial assistance may include, but is not limited to:

  • Travel assistance related to CHD medical care
  • Medical bills related to CHD and not covered by insurance
  • CHD specialized equipment not covered by insurance

Applications are reviewed and awarded on a quarterly basis by a Third-Party Committee.

The third-party committee reserves the right to approve or deny applications as they deem appropriate.

Reimbursement applications are for travel and medical needs. All applications require appropriate requested documentation.

The terms of approval for applications are subject to change at any time.

Applications deadlines are as follows:

  • March 31st
  • June 30th
  • September 30th
  • December 31st

“Fifty years ago we would have known her heart was the problem but have not been able to repair it. Fifteen years ago the heart/lung bypass machine was not advanced enough and would have destroyed her kidneys during her surgery.
